First Appointment
We welcome you to Hugo Madeira Clinic
At your first appointment at the Hugo Madeira Clinic, we want you to feel comfortable and to understand that we are fully committed to eliminating any concerns that may have led to your visit.
We will do a complete analysis of your health condition so that we can give you a correct diagnosis, as well as explain to you the way we work.

1. Check-in
At the reception of the Hugo Madeira Clinic, you will be asked the following:
- Citizen ID card, so that we can create a medical record in our system;
- That you fill out a form with all your relevant clinical data;
- That you sign the declaration regarding the General Data Protection Regulation (GDPR);
- That authorizes radiological protection.
Then you will move to an office with our Smile Manager. This professional has diverse knowledge about dentistry and will understand your goals, clearing up all your doubts.
2. The reality you see
A photo session occurs at the Hugo Madeira Clinic‘s studio in the second stage. The clinical process for correct functional, esthetic, and Oral Health analysis begins.
We need to know all the details of your mouth, and these photographs are a fundamental tool for clinical evaluation. They reveal the interior of the oral cavity and allow a correct observation of the smile, occlusion, symmetries, and body posture.
At the end of the treatment, with a new photo session, it is possible to compare the aesthetic difference between the 2 phases – the so-called “before and after”.

3. The reality that hides
After the photo session, you will see your smile as you have never seen it before, and you will more easily understand the description of the diagnosis presented by the Dentist. What follows is a closer look to see what was not seen with the naked eye.
Some radiographic examinations are performed to learn more about the patient’s clinical history. The Hugo Madeira Clinic has all the necessary equipment to perform these exams.
This step may not be necessary if you have recently performed tests with you.
4. Smile planning
Depending on the case, it may be justifiable to use smile design technology to preview the final smile. To do this, an intraoral scan is performed in the doctor’s office, which takes 3D images of your oral cavity. Thus, it is possible to digitally plan your smile, constantly considering all aesthetic and functional parameters.
At Hugo Madeira Clinic, we consider it essential to manage expectations so that there is no disappointment with the result, and this step can help in that process.

5. In the medical office
In a consultation of about 45 minutes, you can ask all your questions and find out details about your current medical situation. The proposed treatment plan is presented to you and explained in detail.
You are presented with the photographs from the first photo session and the imaging data. Whenever necessary, doctors from other specialities can explain more specific details.
The medical goal of this consultation is, above all, your informed clarification and the delineation of the sequence of treatment priorities. The medical team will pass all treatment plan information to the Smile Manager, who will provide you with all treatment information and suggestions.
6. Presentation of payment methods
After the medical consultation, Smile Manager presents you with the budget for the treatment plan.
The Hugo Madeira Clinic complies with the provisions of the Diário da República, 2nd Series -Nº161 – August 23, 2011, art. 1, point 1, and art. 3, which aims at the mandatory use and compliance with theTANOMD – Nomenclature Table of the Order of Dentists. That is, its provisions govern all budget treatment designation codes.
Here, you will have the opportunity to clarify doubts about payment methods and possible payment facilities and schedule the beginning of your treatments.

What is the purpose of the data collected on the clinical record?
The data collected at the Hugo Madeira Clinic is critical, not only for diagnosis but also for future comparison. Furthermore, it can also be used in article publications or presentations for scientific purposes — if the patient consents to this in writing.
Acting by the provisions of article 30 of the code of ethics of the Portuguese Dental Association, “the dental health care provider must have an archive containing all the individual clinical files of patients, regardless of their format, which must be kept for a minimum period of 20 years”, where “the individual clinical file comprises the clinical record and all the auxiliary means of diagnosis, including photographs, models, imaging exams and all sorts of patient information gathered during the diagnosis and treatment period”. In addition, “the dentist must develop and keep up to date an individual clinical file for the patient, containing, in detail, besides the identification of the dentist who carried out the treatment, personal data, health history, clinical observations, diagnosis, treatment plan and treatments carried out, expressed whenever possible using the Portuguese Dental Association Table of Nomenclature as a reference”.
The Hugo Madeira Clinic also follows all data protection under Portuguese law and is registered with the CNPD – National Commission for Data Protection, with authorisation nº 13825/2017 for process nº 6075/2014.
Can I have a teleconsultation?
Yes, you can have a teleconsultation with one of the clinicians from the medical team responsible for diagnostics and planning consultations.
During the teleconsultation, clinical questions can be clarified. Eventually, a provisional treatment plan can be developed, allowing your Smile Manager to provide you with the estimated values for your treatment.
According to Article 19 of the code of ethics of the OMD, “consultation by telemedicine does not replace the patient-dentist face-to-face contact, but should be carried out under conditions similar to a face-to-face visit”. However, since the Dentist, when performing telemedicine services, does not personally examine the patient, they are always conducting a pre-assessment and never a definitive one.
Is it possible to get information on the price of the procedures by telephone?
It depends. Values are available on the Payments and Financing page and can be communicated by telephone.
However, it would be advisable to have a face-to-face assessment or teleconsultation with one of the clinicians in the medical team responsible for diagnostic and planning consultations to be provided with concrete figures.
